What to Bring

Before visiting Carter Clinics, please bring any relevant medical documents, identification, insurance information, current medication details, and referral forms you may have. You may also wish to bring a list of your symptoms, medical history, allergies, and questions for your healthcare provider to help make your appointment as productive as possible.

  • A valid government-issued photo ID
  • Your Ontario health card and insurance information, if applicable
  • A list of your current medications, vitamins, and supplements
  • Relevant medical records, test results, or referral documents
  • A list of known allergies and existing medical conditions
  • Details of your symptoms and when they began
  • A list of questions you would like to discuss with your healthcare provider

Valuables

We recommend bringing only the personal belongings needed for your appointment. Please keep valuables such as cash, credit cards, jewelry, and electronic devices secure and with you at all times. Carter Clinics is not responsible for lost, stolen, or damaged personal property brought onto the premises.

Be involved in your health care

Patients and healthcare providers work together to achieve better health outcomes. Here are five practical ways you can take an active role in your care:

  • 1. Take an active role in your healthcare and ask questions whenever you need clarification about your condition or treatment.
  • 2. Tell your healthcare provider about previous illnesses, existing medical conditions, surgeries, and relevant family medical history.
  • 3. Bring an up-to-date list of all medications, vitamins, and supplements you currently take.
  • 4. Inform your healthcare provider about any allergies or previous reactions to medications, food, or treatments.
  • 5. Before leaving your appointment, make sure you understand your treatment plan, medication instructions, follow-up requirements, and recommended next steps.

Patient meals

Carter Clinics is an outpatient healthcare facility and does not provide inpatient accommodation or scheduled meal services. Patients are encouraged to eat and stay hydrated before their appointment unless they have received specific fasting or dietary instructions from their healthcare provider.

If your appointment requires fasting, testing preparation, or changes to your usual diet or medication schedule, Carter Clinics will provide the relevant instructions in advance. Please follow these directions carefully and contact our team if anything is unclear.

General appointment preparation guidelines:

  • Confirm whether your appointment requires fasting or special preparation.
  • Follow all medication instructions provided by your healthcare provider.
  • Contact Carter Clinics before your appointment if you have questions about food, drinks, or preparation requirements.
